Doctor D -- way in the back, and absolutely furious: "All right, I'll tell you! The problem is very simple. I know all the doctors in this row, okay? They're named Tom and Bill and Tony and so on. And in front of patients I call them Tom and Bill and Tony and so on! Why should I have to call the women by their last names, for crying out loud? Why do they have to get special treatment? I thought their beef was always about being treated exactly the way everybody else gets treated! It doesn't make any sense!"

This is the kind of thing that drives many men absolutely nuts. FWIW, there is a good answer.
